Lamictal, also known by its generic name Lamotrigine, is a medication used to treat epilepsy and bipolar disorder. It is also used for unipolar depression and to prevent migraine headaches. It is an anticonvulsant, meaning it works to reduce the frequency and intensity of seizures, as well as stabilize mood swings associated with bipolar disorder.

Lamictal (Lamotrigine) is a prescription medication used in the treatment of mental health conditions such as epilepsy and bipolar disorder. Lamotrigine has been found to be effective in the off-label treatment of rapid-cycling bipolar depression, bipolar disorder type I maintenance, and prophylaxis for basilar migraine with aura, panic disorder, and binge eating disorder.

Lamotrigine is an antiepileptic drug that has been approved by the United States Food and Drug Administration (FDA) for the treatment of primary generalized tonic-clonic seizures, including simple and complex partial seizures and focal-onset tonic-clonic seizures, as well as Lennox-Gastaut Syndrome.

Lamotrigine is available as orally disintegrating tablets in 25 mg, 50 mg, 100 mg, and 200 mg strength. When administering lamotrigine, special consideration should be taken if used in conjunction with drugs such as carbamazepine, phenytoin, phenobarbital, primidone, rifampin, lopinavir/ritonavir, atazanavir, ritonavir, and valproic acid, as dose adjustment may be necessary.

Lamictal doses depend on the condition being treated and on whether it is used alone or in combination with other antiseizure medications.

  • For Seizures patients not concurrently taking carbamazepine, phenytoin, phenobarbital, primidone, rifampin, lopinavir/ritonavir, atazanavir, ritonavir, or valproic acid, Lamotrigine should be administered at a dose of 25 mg daily for the first week. At week three, the dose should be increased to 50 mg per day, and at week five, it should be increased by an additional 50 mg each week or every other week until a typical maintenance dose of 225 mg to 375 mg daily in two divided doses is reached.
  • When Lamotrigine is used concurrently with valproic acid, dosing instructions should be followed accordingly in case of Seizures treatment. Initially, the dose should be set at 25 mg every other day. At week three, the dose should be increased to 25 mg daily. At week five, the dose should be increased by an additional 25 mg per week or every other week. The typical maintenance dose for use with valproic acid alone is 100 mg to 200 mg daily in one or two divided doses, while when used with medications that induce glucuronidation, the typical maintenance dose is 100 mg to 400 mg daily in one or two divided doses.
  • When using Lamotrigine in combination with carbamazepine, phenytoin, phenobarbital, primidone, rifampin, lopinavir/ritonavir, atazanavir, or ritonavir to treat Seizures, the following dosing instructions should be followed. Initially, the dosage should be 50 mg given daily. At the third week, the dosage should be increased to 100 mg per day in two separate doses. At the fifth week, it should be increased by an additional 100 mg each week or every other week. The typical maintenance dosage ranges from 300 mg to 500 mg in two separate doses.
  • The recommended dosage of Lamictal (Lamotrigine) for bipolar 1 disorder is 200-400 mg per day, divided into two doses. For people with hepatic disease, the recommended dosage is 50-200 mg per day, divided into two doses. It is important to consult with a healthcare provider before starting or changing the dosage of Lamictal, as the dosage may need to be adjusted based on individual factors.

How Long Does It Take for Lamotrigine to Work?

It is especially important to tell your doctor how you feel things are going during the first few weeks after you start taking lamotrigine. It may take several weeks to see big enough changes in your symptoms to decide if lamotrigine is the right medication for you.

Are There Specific Concerns About Lamotrigine and Pregnancy?

If you are planning to become pregnant, notify your healthcare provider so that he/she can best manage your medications. People living with bipolar disorder who wish to become pregnant face important decisions. It is important to discuss the risks and benefits of treatment with your doctor and caregivers.
